Post-Acute Care NP/PA Opening Adrian, MI
Full Time | Nurse Practitioner or Physician Assistant
Seeking a quality-driven Nurse Practitioner or Physician Assistant to join a post-acute care team serving skilled nursing facilities in Adrian, Michigan.
Position Highlights:

Responsibilities:
- Perform comprehensive patient assessments, including medical histories and physical examinations
- Develop, implement, and adjust individualized treatment plans
- Monitor patient progress and coordinate care across the healthcare team
- Collaborate with physicians, nursing staff, therapists, and other healthcare professionals
- Educate patients and families on disease management and preventive care
- Maintain accurate and timely medical documentation
Qualifications:
- Graduate of an accredited Nurse Practitioner or Physician Assistant program
- Active or eligible Michigan NP or PA license
- DEA certification
- Experience in post-acute care, acute care, emergency medicine, or outpatient practice preferred
- Strong communication, teamwork, and clinical decision-making skills
Compensation & Benefits:
- Estimated compensation of $154,000-$183,000 annually
