Position Overview
We are seeking a compassionate and experienced Advanced Practice Registered Nurse (APRN) to join a well-established Primary Care / Family Practice in Jacksonville, Florida. This is an outstanding opportunity to practice in a collaborative, patient-centered environment focused on preventive care, chronic disease management, and improving the overall health of the community.
The ideal candidate is comfortable managing patients across the lifespan, possesses strong clinical decision-making skills, and enjoys building long-term relationships with patients and families.
Job Responsibilities
• Diagnose and treat acute illnesses, injuries, and chronic medical conditions.
• Develop individualized, evidence-based treatment plans.
• Order, interpret, and review laboratory tests, diagnostic studies, and imaging.
• Prescribe medications and monitor treatment effectiveness.
• Manage chronic diseases including:
o Hypertension
o Diabetes
o Hyperlipidemia
o COPD
o Asthma
o Thyroid disorders
o Obesity
o Heart disease
• Provide preventive care, immunizations, and health screenings.
• Perform Medicare Annual Wellness Visits.
• Educate patients on disease prevention, nutrition, exercise, and lifestyle modifications.
• Coordinate referrals to specialists and collaborate with multidisciplinary healthcare providers.
• Maintain accurate and timely documentation in the electronic medical record (EMR).
• Participate in quality improvement initiatives and population health programs.

Qualifications
• Current Florida APRN license (or eligibility for licensure).
• National Board Certification as a Family Nurse Practitioner (FNP) or Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP).
• Current DEA registration (or ability to obtain).
• Minimum of 2 years of Primary Care or Family Practice experience preferred.
• Excellent communication and patient education skills.
• Strong commitment to evidence-based, patient-centered care.
