**Nurse Practitioner Opportunity in Sandusky, OH**
**Position Overview:**
Seeking a Nurse Practitioner to join a multidisciplinary healthcare team providing evidence-based care for acute and complex chronic conditions. This full-time position primarily focuses on outpatient care with potential involvement in inpatient coordination, offering an opportunity to practice in a collaborative, patient-centered environment.
**Practice Details:**
- Full-time employed position
- Primarily outpatient practice
- Collaborate with physicians and multidisciplinary care teams
- Coordinate inpatient care and discharge planning as needed
- Evidence-based clinical practice
- Utilize established clinical pathways and protocols
**Clinical Responsibilities:**
- Perform comprehensive patient evaluations and physical examinations
- Diagnose and manage acute and chronic medical conditions
- Order, interpret, and manage diagnostic testing
- Develop individualized treatment plans
- Prescribe medications and therapeutic interventions
- Perform office-based procedures
- Manage complex wound care and minor procedures
- Coordinate care across the continuum
- Maintain timely and accurate EMR documentation
- Participate in quality improvement initiatives
**Requirements:**
- Master's degree from an accredited Nurse Practitioner program
- Active Nurse Practitioner license
- DEA or eligibility to obtain
- Eligible for credentialing
- BLS certification
- ACLS certification if required by assigned practice
- Minimum of two years of experience preferred
- Strong clinical decision-making and communication skills
