#### **Candidate Qualifications:**
- Board Certified **Nurse Practitioner (NP)** or **Physician Assistant (PA)**
- Active and unrestricted **New York License**
- Active **DEA Registration**
- Post-acute care experience preferred, but not required
- Strong communication and collaboration skills

- **Estimated Base Salary:** $130,760 – $154,911 annually
