We are seeking a dedicated Family Nurse Practitioner (FNP) to join a well-established outpatient clinic in San Jose, CA. This role focuses on delivering high-quality, patient-centered care across a diverse population, with an emphasis on preventive care, health education, and chronic disease management.
This is an excellent opportunity for an FNP looking to work in a collaborative, community-focused setting with strong clinical support and a balanced schedule.
Job Responsibilities
• Conduct physical exams, wellness visits, and preventive screenings
• Diagnose and treat acute and chronic conditions
• Manage chronic diseases such as diabetes, hypertension, and asthma
• Prescribe medications and treatments as appropriate
• Educate patients and families on health maintenance and disease prevention
• Maintain accurate EMR documentation
• Collaborate with physicians and healthcare team members
Qualifications
• Board Certified Family Nurse Practitioner (FNP-C / FNP-BC)
• Active California NP License (or ability to obtain)
• Active DEA License
• BLS Certification
• Prior outpatient experience preferred
• Strong communication and cultural competency skills
