Required Skills:
- Valid Kentucky Medical License
- Certified Nurse Practitioner qualification in Family Practice
- Proficiency in telehealth technology and electronic health records
- Strong assessment and critical thinking skills
- Ability to perform risk assessments and chronic condition reviews
- Excellent communication and interpersonal skills
- Comfortable seeing both pediatric and adult patients
Nice to Have Skills:
- Experience with home health visits
- Familiarity with Medicaid and Medicare processes
- Prior telehealth experience
- Bilingual abilities
Preferred Education and Experience:
- Master’s degree in Nursing (MSN) or higher
- Minimum of 2 years of Family Practice experience
- Prior experience working in telehealth or home health settings
Other Requirements:
- Enrolled and credentialed in Medicard (confirmation required upon application)
- Ability to manage a schedule of approximately 10-14 patients daily (telehealth and in-home mix)
- Willingness to perform risk assessments and follow established protocols
- Reliable internet connection and access to equipment mailed by the client
- Flexibility to work day shifts, Monday through Friday, 8:00 am to 5:00 pm, with scheduled days released 2-3 weeks in advance
- Paid orientation (1-3 hours) plus ongoing education during downtime
- Travel within a 30-mile radius for home health visits as needed
