Required Skills:
  • Valid Kentucky Medical License
  • Certified Nurse Practitioner qualification in Family Practice
  • Proficiency in telehealth technology and electronic health records
  • Strong assessment and critical thinking skills
  • Ability to perform risk assessments and chronic condition reviews
  • Excellent communication and interpersonal skills
  • Comfortable seeing both pediatric and adult patients

Nice to Have Skills:
  • Experience with home health visits
  • Familiarity with Medicaid and Medicare processes
  • Prior telehealth experience
  • Bilingual abilities

Preferred Education and Experience:
  • Master’s degree in Nursing (MSN) or higher
  • Minimum of 2 years of Family Practice experience
  • Prior experience working in telehealth or home health settings

Other Requirements:
  • Enrolled and credentialed in Medicard (confirmation required upon application)
  • Ability to manage a schedule of approximately 10-14 patients daily (telehealth and in-home mix)
  • Willingness to perform risk assessments and follow established protocols
  • Reliable internet connection and access to equipment mailed by the client
  • Flexibility to work day shifts, Monday through Friday, 8:00 am to 5:00 pm, with scheduled days released 2-3 weeks in advance
  • Paid orientation (1-3 hours) plus ongoing education during downtime
  • Travel within a 30-mile radius for home health visits as needed

Compensation and Benefits:

Hourly Rate: $95.48
