APP Part Time - Primary Care Network, Waterford Lakes

AdventHealth Primary Care+ is seeking a Part-Time Advanced Practice Provider to join our team-based primary care model. This opportunity is designed for providers who value whole-family care, continuity with patients, and a collaborative practice environment that supports both high-quality care and work–life balance.

In this role, you will work closely with other Advanced Practice Providers and Care Teams to deliver comprehensive primary care in a modern, well-supported clinic focused on preventive care, chronic disease management, and long-term patient relationships.

Schedule Details

This position follows a fixed 4-week rotating schedule. All listed shifts are required each week.

• Weeks 1–2: Thursday and Friday, 8:00 a.m.–8:00 p.m.
• Week 3: Friday, 8:00 a.m.–8:00 p.m.; Saturday, 8:00 a.m.–5:00 p.m.
• Week 4: Sunday, 8:00 a.m.–5:00 p.m.; Thursday, 8:00 a.m.–8:00 p.m.; Friday, 8:00 a.m.–2:00 p.m.

Qualifications
• Active Florida APRN or PA-C license
• Primary Care or Family Medicine experience (preferred)
• Strong skills in preventive care and chronic condition management
• Ability to work collaboratively within a team-based care model
• BLS Required
