- Start Date: April 13, 2026
- Contract Length: 3 months (option to extend)
- Full-time: 40 hours/week, Monday–Friday, 8 AM–5 PM (1-hour lunch)
- Part-time: 20 hours/week available
- Schedules Released: 2–3 weeks in advance
- Paid: Full day pay regardless of patient volume (requires availability input in Salesforce)
- Orientation: 1–3 hours paid; ongoing education during downtime
- Telehealth: ~70% of time
- 12–14 patients per telehealth day
- In-Home Visits: ~30% of time (Milwaukee area)
- 6–7 visits per in-home day
- Mixed Schedule: About 10 visits per day
- See both pediatric and adult patients (must be comfortable with both)
- Assessment-only role — no prescribing or treatment
- Perform annual risk assessments and chronic condition reviews
- Escalate urgent issues to patient’s PCP
- Must be enrolled and credentialed in Medicard (confirm and highlight in submission)
- Open to full-time or part-time providers
- Referral fees available for qualified candidate referrals
