We are seeking a dedicated and skilled Family Practice Nurse Practitioner to provide compassionate primary care services across Oregon, including Lane, Multnomah, and Washington counties. This pivotal role offers the opportunity to deliver holistic health assessments while enjoying a flexible schedule and comprehensive benefits. Whether you prefer full-time or part-time engagement, you will play a vital role in enhancing community health through a blend of telehealth and home health visits.

Required Skills:
  • Active Oregon Medical License
  • Enrolled and credentialed in Medicard
  • Experience in primary care settings with pediatrics and adult patients
  • Strong communication skills and ability to assess both physical and psychosocial health
  • Comfort with telehealth platforms and in-home visit procedures
  • Ability to perform risk assessments and chronic condition reviews
  • Self-motivated with excellent time management

Nice to Have Skills:
  • Prior experience with home health care
  • Bilingual abilities in Spanish or other languages
  • Familiarity with electronic health record systems such as Salesforce
  • Additional certifications in advanced practice nursing or chronic disease management

Preferred Education and Experience:
  • Master’s Degree or higher in Nursing (MSN or DNP)
  • Minimum 1-3 years of experience in family practice or primary care
  • Experience providing telehealth services and home health care is highly desirable

Other Requirements:
  • Willingness to travel within designated areas, with mileage reimbursed at IRS standard rates
  • Availability for 8 am-5 pm shifts, Monday through Friday
  • Ability to see approximately 12-14 patients per day via telehealth, or 6-7 in-home visits; combined schedules average around 10 visits daily
  • Proactive participation in orientation and ongoing education modules during work hours
  • Schedule released 2-3 weeks in advance, with flexibility in part-time or full-time allocations
