- Schedule: Monday-Friday 8:00 a.m.-5:00 p.m.
- Patients Per Day: 35
- Practice Setting: Health Center
- Scope: Primary Care, All Ages
- State Licensure: Active Arizona State License and Drug Enforcement Administration (DEA)
- Credentialing Timeframe: 2 Weeks
