| Modality: | Family Nurse Practitioner |
| Openings: | 1 FNP Fortuna, CA AND Ferndale, CA This assignment will have the provider time split between two sites about 15 minutes apart. They will work 1 - 2 days per week at Ferndale Center and the other days at the Fortuna Center. |
| Shifts: | NO CALL |
| Shift Days: | Mon-Fri 8am-5pm |
| Hours per Week: | 40 |
| Contract Length: | Provider must be willing to work 3 months minimum with the option to extend - ongoing need Provider must provide 30-day notice to end any contracts |
| Start Date: | ASAP start once credentialed (takes 30-60 days) 4-6 weeks to credential is standard for locums who submit application/documents in timely |
| Details of Assignment: | - Family medicine primary care services needed for outpatient clinic for patients of all ages - common visits: primary care of acute illnesses (sick, sore throat, other URI, UTI, burns, cuts, etc), chronic condition management of multiple comorbidities (diabetes, HTN, HLD, CHF, etc), and some chronic pain management - 50/50% kids/adults - expectation of minimum 18 patients per day - Chart closure within 72 hours required - TRAVEL/LODGING covered by client (flight, rental car OR mileage reimbursement if personal car and lodging); lodging may be available in-house with facility |

| Minimum Experience and Certification Required: | - Must be licensed in California - Must have BLS - Must have active DEA before the start date - Must be board-certified as an FNP through ANCC or AANP - Must have minimum of 3 years of experience in a rural health setting, community health center, or FQHC |
