Location: Adell, Wisconsin (Milwaukee area for home visits)
- Start Date: April 6th or ASAP
- Assignment Length: Month-to-month with option to extend
- Schedule:
- Monday–Friday
- 8:00 AM – 5:00 PM
- 40 hours/week (Full-time only)
- Schedule Release: 2–3 weeks in advance
- Call Requirements: None
- Board Certification: Family Practice – Required
- State License: Active Wisconsin License – Required
- Willing to License: No
- Must be enrolled/Credentialed in Medicard
- Patient Population: Must be comfortable seeing Adults & Pediatrics
- Care Model:
- 70% Telehealth
- 30% Home Health (within ~30-mile radius)
- Volume:
- Telehealth: 12–14 patients/day
- Home Visits: 6–7 patients/day
- Mixed: ~10 patients/day
- Perform annual risk assessments and chronic condition reviews
- Assessment-only role (no prescribing or treatment required)
- Escalate urgent findings to patient’s PCP
- Orientation: 1–3 hours (paid)
- Ongoing Modules: Completed during working hours as downtime allows
- Equipment: Provided and mailed to provider
- Home Visit Area: Milwaukee region
