APP (NP / PA) – In-Home Chronic Care

What You’ll Do
As an Advanced Practice Provider, you’ll provide comprehensive in-home care and care coordination for medically complex patients.
? Conduct initial assessments, follow-ups, and hospital transition visits
? Perform full vitals and head-to-toe physical exams
? Manage medications and chronic care plans
? Provide patient education on chronic disease management
? Collaborate in team rounds discussing high-risk patients
? Document visits in Athena with real-time care team visibility
Daily Patient Load:
• 4–5 visits per day
• New patient visits: ~90 minutes
• Follow-ups: 30–60 minutes
Schedule & Flexibility
Hybrid Schedule
3–4 days: In-home patient visits
1–2 days: Remote admin, charting, or telehealth
No nights
No weekends
No on-call

Compensation & Benefits
Base Salary: $120,000 – $145,000 (DOE)

Qualifications
Nurse Practitioner or Physician Assistant license
2+ years of Advanced Practice experience
Experience managing care plans or chronic disease patients preferred
