Family Practice Nurse Practitioner

As a vital member of our team, you'll deliver comprehensive assessments, conduct risk evaluations, and support patient well-being—all within a supportive, professional setting that values your expertise.

The ideal candidate will possess strong clinical skills, excellent communication abilities, and a passion for delivering compassionate care across pediatric and adult patients. This is a unique chance to work in a role that emphasizes assessment and coordination, with no treatment or prescribing required, enabling a focus on patient evaluation and escalation.

Required Skills:
  • Active Michigan Medical License
  • Enrollment and credentialing in Medicard
  • Proficiency in telehealth technology platforms and electronic health records
  • Excellent communication and interpersonal skills
  • Ability to perform risk assessments and chronic condition reviews
  • Comfort seeing pediatric and adult patients
  • Reliable transportation within a 30-mile radius for home health visits

Nice to Have Skills:
  • Prior experience in telehealth or home health settings
  • Bilingual abilities
  • Familiarity with Salesforce or similar scheduling tools
  • Additional certifications related to primary care or health assessments

Preferred Education and Experience:
  • Master’s Degree in Nursing (MSN) or higher
  • At least 2 years of clinical experience in primary care or community health environments
  • Experience with pediatric and adult patient populations

Other Requirements:
  • Willingness to participate in scheduled telehealth and home health visits
  • Ability to complete ongoing education modules during work hours
  • Availability for scheduled shifts Monday through Friday, 8 am-5 pm
  • Equipment will be provided and mailed to the provider
  • Paid orientation and training (1-3 hours), with ongoing education during downtime

This full-time position, starting on April 6, 2026, offers a competitive hourly pay rate of $95.48.
