Required Skills:
- Active Michigan Medical License
- Enrollment and credentialing in Medicard
- Proficiency in telehealth technology platforms and electronic health records
- Excellent communication and interpersonal skills
- Ability to perform risk assessments and chronic condition reviews
- Comfort seeing pediatric and adult patients
- Reliable transportation within a 30-mile radius for home health visits
Nice to Have Skills:
- Prior experience in telehealth or home health settings
- Bilingual abilities
- Familiarity with Salesforce or similar scheduling tools
- Additional certifications related to primary care or health assessments
Preferred Education and Experience:
- Master’s Degree in Nursing (MSN) or higher
- At least 2 years of clinical experience in primary care or community health environments
- Experience with pediatric and adult patient populations
Other Requirements:
- Willingness to participate in scheduled telehealth and home health visits
- Ability to complete ongoing education modules during work hours
- Availability for scheduled shifts Monday through Friday, 8 am-5 pm
- Equipment will be provided and mailed to the provider
- Paid orientation and training (1-3 hours), with ongoing education during downtime
