Job Details
- Employment Type: Locum tenens / contract (travel Nurse Practitioner, home health)
- Location: Southeast Wyoming – Albany, Platte, Goshen, Laramie counties (base zip 82201, Wheatland, WY)
- Role Focus: In-Home Health Risk Assessments, geriatric assessments, medication reviews
- Shift Duration: 8-hour field days (including travel)
- Schedule: Flexible start (7:00 AM or 8:00 AM); preferably Mon–Fri

- Travel Requirements: >50% travel (up to 120 miles/day depending on territory) – travel nurse practitioner / mobile NP duties

Job Requirements / Qualifications
- Active, unrestricted Nurse Practitioner license in Wyoming (required)
- Board certification: ANCC, AANP, or AACN
- Minimum 1 year of Nurse Practitioner clinical experience (no new grads)
- Experience with geriatric patients, home health, or community-based assessments preferred
- Comfortable traveling alone to member homes, including rural and underserved areas
- Adaptable to route changes, no-shows, and schedule unpredictability
- Strong patient-centered care focus and reliable, professional bedside manner
- Must provide your own patient bag with basic medical tools
- Dress code: Business professional or dark-colored scrubs (no lab coat required)
Key Responsibilities
- Perform annual in-home Health Risk Assessments for members (one-on-one visits; no follow-up required)
- Conduct focused physical exams, medication reviews, and assess social & environmental determinants of health
- Complete basic point-of-care tests (e.g., A1C, fit tests – no venipuncture required)
- Document and submit assessments same-day via a Matrix-issued tablet (accurate, timely charting required)
- Travel daily to member homes, covering up to 120 miles per day; manage logistics for rural territories
- Manage schedule changes, cancellations, and no-shows while maintaining patient access and quality metrics
- Maintain high clinical quality scores and adhere to clinical review and documentation processes
- Communicate and collaborate with the Clinical Services team for support and quality assurance
