A growing value-based primary care organization is seeking a Family Nurse Practitioner or Adult-Gerontology Nurse Practitioner to join a collaborative outpatient clinic in New Britain, Connecticut.
This practice model focuses on delivering high-quality care to Medicare and geriatric patient populations through smaller patient panels, longer appointment times, and a strong interdisciplinary support team.

• Clinic hours: 8:00 AM – 5:00 PM
• 40 hours per week
• Outpatient primary care setting
• Patient visits scheduled 30 minutes each
• Patient panel capped at ~450 patients per provider
• Minimal call (shared statewide rotation)

• Family Nurse Practitioner (FNP) or Adult-Gerontology NP
• Board Certified (ANCC or AANP)
• Active Connecticut NP license
• Active DEA license
• Experience in primary care, internal medicine, geriatrics, or family medicine preferred
New graduates may be considered.
