The Family Nurse Practitioner / Advanced Practice Provider at Pinon Health Center delivers comprehensive primary and acute care across the lifespan, managing both chronic and acute medical conditions. They perform patient assessments, develop diagnoses, prescribe medications, and create individualized care plans while coordinating with multidisciplinary healthcare teams. The provider educates patients, promotes preventive health, and ensures continuity of care, including referrals to specialty services when needed. This role requires adherence to clinical protocols, federal healthcare regulations, and a commitment to patient-centered, culturally competent care.
Job Responsibilities
- Develop differential and definitive diagnoses using clinical knowledge and judgment
- Order, interpret, and evaluate laboratory tests, imaging, and other diagnostic procedures
- Prescribe medications and manage pharmacologic treatment plans
- Provide acute care and stabilization for urgent or emergent conditions
- Manage chronic diseases including diabetes, hypertension, respiratory, and metabolic disorders
- Deliver preventive care, wellness screenings, and health maintenance services
- Screen for mental health conditions and provide initial assessment or referral
- Counsel patients on treatment plans, lifestyle changes, and medication adherence
- Coordinate patient care with multidisciplinary healthcare teams and external specialists
- Ensure continuity of care through follow-ups and ongoing patient monitoring
- Document clinical findings and care plans accurately in electronic health records
- Follow clinical protocols, federal regulations, and IHS policies for patient safety
- Participate in quality improvement initiatives and patient satisfaction programs
- Provide culturally competent, patient-centered care that addresses social determinants of health
Qualifications
- Certified Family Nurse Practitioner (NP) or Advanced Practice Nurse (APN) credential
- Valid, unrestricted Nurse Practitioner or Advanced Practice license throughout the contract term
- Board certification with an approved Family NP credentialing organization
- Minimum of 2 years of in-person clinical experience as a Family NP or APN
- At least 6 months direct face-to-face experience in the specialty required by the contract
- No lapse in clinical service exceeding three consecutive months within the prior three months
- Valid, current DEA license
- Current CPR / BLS / PALS certifications; ACLS and ATLS highly recommended
- National Practitioner Data Bank (NPDB) report available
- Three professional references supervisors or healthcare providers familiar with clinical practice
- Experience providing primary care, acute care, preventive care, and chronic disease management
- Strong patient communication, clinical judgment, and care coordination skills
- Commitment to patient-centered, culturally competent care and adherence to federal regulations
Working Hours
8:00 AM – 5:00 PM (regular tour of duty)
Total: 40 hours per week
Additional details- 3-6 month contract
The Pinon Health Center is closed on Federal Holidays.
Providers are not compensated for federal holidays when the clinic is closed.
