Full-time or Part-time post-acute care role for an NP or PA providing primary care services in skilled nursing facilities. This position offers strong clinical support, autonomy, and uncapped earning potential in a collaborative care model.
Position Highlights
• Full-time or Part-time rounding role – (M-F)
• Post-acute care in skilled nursing facilities
• No primary night call

Key Responsibilities
• Perform patient assessments, H&Ps, and care planning
• Manage acute and chronic conditions
• Coordinate care with physicians and interdisciplinary teams
• Educate patients and families
• Maintain timely and accurate documentation

• Estimated annual compensation: $138,334–$154,911 (ft) (uncapped productivity)
