This role is with an established outpatient primary care organization that focuses on providing value-based care to local Medicare patient populations. Providers here work within large, well-supported clinical teams that emphasize delivering high-quality care rather than high-volume patient turnover.
Position Highlights:
- Schedule: Monday–Friday, 8am–5pm (no weekends)
- Setting: Outpatient primary care
- Patient Population: Primarily older adults/geriatric patients
- Support: Each provider is paired 1:1 with both a scribe and an MA; RN support is shared (1 RN per 2–3 providers)
- Visit Length: All appointments are 30 minutes
- Panel Size: Capped at about 450 patients to maintain quality
- Call: Minimal—rotated among all providers statewide
- Start Date: ASAP after credentialing (approx. 60 days)
- Charting: EPIC (currently being implemented)
Requirements:
- Board-certified FNP (AANP or ANCC)
- Active Pennsylvania NP license
- Active DEA
- New grads are welcome
- Preferred: experience in primary care and familiarity with HCC documentation, ICD-10 coding, and Health Risk Assessments
- Passion for team-based care and a population-health model
