Family Medicine Advance Practice Provider (APP) - Chicago, IL

Work Schedule

• Full-time or part-time opportunities available
• Clinic-based schedule with dedicated administrative time
• Occasional weekend coverage may be required

Patient Population
• Family Medicine patients including pediatric and adult populations

Position Overview
• Provide primary medical care services to pediatric and adult patients in an outpatient primary care setting
• Focus on preventive care, early diagnosis, and promotion of healthy lifestyles
• Work collaboratively with physicians, nurses, and healthcare staff to deliver high-quality patient care
• Participate in clinical programs and community health initiatives when required

Responsibilities
• Obtain patient histories and perform physical examinations
• Diagnose medical conditions and provide evidence-based treatment plans
• Prescribe medications and order diagnostic tests as appropriate
• Provide patient education on preventive care and health management
• Maintain accurate and timely medical documentation
• Participate in quality improvement activities, chart audits, and peer review processes
• Manage follow-up appointments and referrals to ensure continuity of care
• Implement preventive health schedules for pediatric and adult patients
• Maintain clinical competency through continuing education and professional development
• Support training and mentoring of healthcare staff when required
• Participate in community health education and outreach programs

Qualifications
• Graduate of an accredited nursing program
• Completion of an accredited post-graduate training program
• Active state license, controlled substance license, and DEA registration
• Bilingual English and Spanish required
• Experience providing direct patient care as an Advanced Practice Provider preferred
• Excellent interpersonal and collaboration skills
• Strong organizational and time management abilities
• Ability to prioritize tasks and manage multiple responsibilities
