Nurse Practitioner (Adult Medicine)- Brooklyn, NY

Facility Type: Direct patient care in a clinical setting
Employment Type: Full-Time (35 paid hours/week)
Schedule: Monday-Friday, some Saturdays

Position Overview:

The Nurse Practitioner (Adult Medicine) provides comprehensive, patient-centered primary care services to adult patients within a federally qualified health center (FQHC) setting. This role focuses on preventive care, chronic disease management, and evidence-based treatment.

The NP functions as an independent primary care provider while collaborating closely with physicians and interdisciplinary teams to deliver high-quality care.

Key Responsibilities:

  • Conduct comprehensive health assessments, histories, and physical examinations
  • Diagnose and manage acute and chronic adult conditions
  • Manage chronic illnesses such as diabetes, hypertension, asthma, and cardiovascular disease
  • Coordinate care with specialists and behavioral health providers
  • Support compliance with FQHC and HRSA standards
  • Assist in tracking productivity and quality metrics
  • Maintain timely and compliant EHR documentation
  • Participate in on-call rotation as required

Required Qualifications:

  • Master's degree or higher from an accredited NP program (Adult, Adult-Gerontology, or FNP with adult focus)
  • Active New York State Nurse Practitioner license
  • National NP certification (AANP or ANCC)
  • DEA registration and NPI (or ability to obtain)
  • Current CPR/BLS certification
  • Minimum 2 years of adult primary care experience
  • Strong diagnostic and clinical assessment skills
  • Experience with electronic health records (HER)
Preferred Qualifications:
  • Experience in an FQHC, community health center, or safety-net setting
