Job description

Nurse Practitioner – Hybrid In-Home Chronic Care
Location: Linn County, IA
Schedule: Full-Time | Monday–Friday | Hybrid (Field + Remote)

Step Away from the Rush. Focus on the Patient.

If you’re a Nurse Practitioner who values autonomy, meaningful patient relationships, and time to truly practice medicine, this role offers a refreshing alternative to the fast-paced clinic or hospital environment.

Join a nationally recognized organization transforming care for patients with chronic kidney disease (CKD) and end-stage renal disease (ESRD) through a technology-enabled in-home care model. Instead of short appointment slots and crowded schedules, you’ll have dedicated time with each patient, delivering thoughtful, high-impact care in the comfort of their home.

This role is ideal for providers who want to slow down, build relationships, and make a measurable difference in chronic disease management.

Why Providers Love This Role

  • Autonomy in your practice while still being supported by a collaborative care team

  • More time with patients – visits are designed to allow meaningful clinical interactions

  • Hybrid schedule with both field and remote days

  • No weekends or on-call responsibilities

  • AI-supported scheduling that clusters visits geographically to reduce drive time

What Your Week Looks Like

Hybrid structure:

  • 3–4 days conducting in-home visits

  • 1–2 days remote for telehealth, documentation, and care coordination

Patient care model:

  • Approximately 4–5 patients per day

  • 90-minute initial visits for comprehensive assessments

  • 30–60 minute follow-up visits

Your day may include:

  • Conducting comprehensive in-home assessments

  • Managing chronic conditions and medications

  • Supporting hospital-to-home transitions

  • Educating patients and caregivers on disease management

  • Coordinating care with interdisciplinary teams

This model allows you to practice at the top of your license while building deeper patient relationships.

Compensation & Benefits

  • Competitive base salary (based on experience)

  • 401(k) with 100% employer match — fully vested day one

  • Generous PTO + paid holidays

  • Company-paid life insurance

  • FSA/HSA options

  • Pet insurance

  • Licensure, malpractice, and CME covered

Growth & Advancement

This is a rapidly growing organization with strong internal mobility. Providers have opportunities to expand into additional specialties including:

  • Nephrology

  • Palliative Care

  • Cardiology

  • Pulmonology

  • Endocrinology

Career progression is competency-based, with pay increases tied to skill mastery and expanded scope.

If you’re looking for a role where you can practice with greater independence, spend more time with each patient, and deliver truly patient-centered care, this opportunity offers the flexibility and support to do just that.
