Nurse Practitioner – Hybrid In-Home Chronic Care
Location: Story County, IA
Schedule: Full-Time | Monday–Friday | Hybrid (Field + Remote)

What Your Week Looks Like

Hybrid structure:

  • 3–4 days conducting in-home visits

  • 1–2 days remote for telehealth, documentation, and care coordination

Patient care model:

  • Approximately 4–5 patients per day

  • 90-minute initial visits for comprehensive assessments

  • 30–60 minute follow-up visits

Your day may include:

  • Conducting comprehensive in-home assessments

  • Managing chronic conditions and medications

  • Supporting hospital-to-home transitions

  • Educating patients and caregivers on disease management

  • Coordinating care with interdisciplinary teams

This model allows you to practice at the top of your license while building deeper patient relationships.
