Job Title: Nurse Practitioner / Physician Assistant (Primary Care)
Location: Phoenix, AZ
Hours & Schedule: Monday – Friday | 7:30 AM – 4:30 PM
Work Environment: Outpatient private practice with three established locations
Salary / Hourly Rate: $120,000 – $150,000 annually

Job Summary:
The Nurse Practitioner or Physician Assistant will provide comprehensive primary care services in an outpatient family medicine setting. This role includes delivering preventive, acute, and chronic care to a diverse patient population while supporting quality initiatives and coordinated, team-based care. The provider will maintain accurate documentation and contribute to ongoing practice-wide quality improvement efforts.
Job Duties & Responsibilities:
- Perform annual wellness exams and provide preventive, acute, and chronic care
- Manage a diverse panel of patients across all ages
- Maintain accurate and timely documentation in the EMR
- Collaborate with clinical staff to ensure coordinated, patient-centered care
- Support managed care initiatives and quality programs
- Provide patient education to promote healthy lifestyle and disease prevention
- Participate in practice-wide quality improvement efforts
Prerequisites / License & Certification Requirements:
- Board Eligible or Board Certified Nurse Practitioner or Physician Assistant
- Valid Arizona medical license (or ability to obtain)
- Current DEA registration
- Experience in outpatient primary care preferred
- Strong patient communication and interpersonal skills
