We are seeking an experienced Family Practice Nurse Practitioner to provide locum services in Adell, WI. Coverage is needed ASAP and ongoing month to month.
Job Details:
- Setting: 70% Telehealth 30% in Home (Home Health)
- Shift/Schedule Details:
- Full-time (40 hours/week) Monday–Friday or Part-time available (20 hours)
8 am-5 pm with a 1-hour lunch - Open to full time or part time providers
- Full-time (40 hours/week) Monday–Friday or Part-time available (20 hours)
- Skills/Procedures/Required Case Types:
- Conduct 5–6 in-home health assessments per day
- Perform annual risk assessments and chronic condition reviews
- Assessment-only role — no treatment or prescribing required
- Escalate urgent findings to the patient’s PCP
- A1C testing only (if applicable; no additional screenings required)
