Job description StaffMed Health Partners is recruiting a full-time Nurse Practitioner (NP) or Physician Assistant (PA) for a post-acute care opportunity in Farmington, New Mexico. This position offers scheduling flexibility, strong infrastructure support, and uncapped earning potential within a well-established post-acute care model.

Practice Highlights/Qualifications:
  • Post-acute care setting serving skilled nursing and long-term care facilities
  • Manage approximately 15–20 patients per day
  • Deliver comprehensive primary care for acute and chronic conditions
  • Develop individualized care plans and coordinate with interdisciplinary teams
  • Collaborate closely with physician partners
  • Required: Board-certified FNP, AGACNP, ACNP, AGPCNP, or PA-C
  • Active New Mexico NP/PA license (or eligibility)
  • Active DEA required
  • Experience in post-acute or long-term care preferred
