Hybrid Primary Care FNP – Bronx, NY (Bilingual Spanish)
A rapidly growing, mission-driven healthcare organization is seeking a Family Nurse Practitioner (FNP) to provide hybrid, home-based primary care services throughout the Bronx. This role is ideal for a provider who is passionate about delivering high-quality, relationship-driven care directly in patients’ homes.
What They’re Looking For:

Bilingual Spanish (medical Spanish proficiency acceptable)

1–2+ years of Nurse Practitioner experience

Comfortable providing primary care in a home-based setting

Strong clinical acumen and patient-centered approach
Schedule:

Monday–Friday
-
No weekends!

Hybrid home-care model (field-based primary care in the Bronx)
Compensation & Benefits:
-
Base salary: $140,000–$172,000
-
Total compensation can exceed $200,000+ at the top of range (including 15% bonus)

Job Responsibilities
Conduct health assessments, physical exams, and develop individualized treatment plans
Diagnose and manage acute and chronic conditions (e.g., HTN, diabetes, CHF, COPD)
Qualifications
Bilingual Spanish (medical Spanish proficiency acceptable)
1–2+ years of Nurse Practitioner experience
Comfortable providing primary care in a home-based setting
Working Hours
HYBRID role - flexibility to work from home on days patients do not need to be seen in person
