Nurse Practitioner – ISNP / Geriatric Primary Care (Travel / Mobile Role)
Location: San Fernando Valley, CA (Burbank → Santa Clarita → West Hills)
Position Type: Travel/Mobile Nurse Practitioner
Setting: Patient homes & long-term care facilities
Start Date: ASAP
Assignment Length: 6 months with possible extension
Schedule: Monday–Friday, 8:00 AM–5:00 PM
Patient Volume: 5–7 patients per day
Role Overview
Amergis Healthcare is seeking a Nurse Practitioner to support an ISNP (Institutional Special Needs Program) within the San Fernando Valley. This role involves traveling throughout the region to provide primary care services to geriatric patients in their homes and/or facility settings.
Requirements
- Certifications:
- Active NP license
- Board Certified
- BLS
- DEA
- Experience:
- Minimum 1 year in geriatric primary care
- Prior mobile/travel NP experience visiting homes or facilities preferred
- Strong knowledge of HEDIS and Value-Based Care (VBC)
- Skills:
- Ability to work independently within a mobile care environment
- Strong communication and clinical assessment skills
Key Responsibilities
- Provide geriatric primary care within patient homes and facility settings
- Conduct assessments, chronic care follow-up, and preventative care
- Collaborate with interdisciplinary teams across ISNP programs
- Support quality metrics and VBC initiatives
