Location: Bronx base (10451), serving all NYC Boroughs
Position: Nurse Practitioner - Family Practice (1 opening)
Provide vital home-based primary care across NYC's boroughs (Manhattan, Bronx, Brooklyn, Queens, Staten Island). Join a team focused on accessible Family Practice for homebound patients.
Key Responsibilities:
- Perform house call visits for assessments, treatments, and preventive care
- Manage chronic conditions, prescribe medications, and order diagnostics
- Collaborate with physicians and multidisciplinary teams via EHR
- Ensure patient safety and compliance during mobile visits
Qualifications:
- Family Nurse Practitioner certification (FNP) with active NY NP license
- DEA licensure and experience in primary/home care preferred
- Reliable transportation and comfort with urban travel
