Job description Position will support the south west region of Cleveland

We provide a wide range of essential services under one roof, including pharmacy, X-rays, blood lab services, nutrition support, urgent care, and more.

Here are some highlights:

• Patient Focus: We prioritize patient outcomes over volume.
Compensation: We offer excellent pay ranging from $150K–$165K, plus incentives.
• Time Off: Enjoy 22 days of PTO, 9 holidays, and 5 days for CME.
• Schedule: No weekends, holidays, or evening shifts.
• Staffing: Full staffing, including RNs, MAs, care teams, social workers, and behavioral health professionals.

Qualifications

Qualifications:
2+ years’ experience as a Nurse Practitioner/Physician Assistant
Familiar with care transitions, strategies for reducing readmissions and chronic condition management interventions
Knowledge of local population, geography, and resources.
Ability to take a creative and innovative approach to problem-solving to aid patients in overcoming barriers to care.
Knowledge and experience using Electronic Medical Records and ability to analyze and leverage their reporting capabilities.
Excellent computer skills, including knowledge of Microsoft Office.
Multiple settings experience (urgent care, ER, inpatient, office, or community based) preferred.
Experience working with high risk and medically complex patients with multiple comorbidities preferred.
Psychiatric and substance use disorder experience preferred.
