  • Established physician-owned multispecialty group seeking a Nurse Practitioner to provide care in local skilled nursing facilities
  • Competitive compensation — discussed directly with the client
  • Full-time position serving SNF patients in the Fall River area
  • Coordinated care model supporting transitions from hospital to SNF to home
  • Focus on efficient, cost-effective care and reducing hospital readmissions
  • Work alongside other NPs and facility medical directors
  • Develop management plans and direct care for SNF patients
  • Must hold active Massachusetts NP license, DEA, and MA controlled substances registration
  • Strong organizational skills, independence, flexibility, and problem-solving abilities required
  • Must collaborate effectively with nurses, therapists, pharmacists, administrators, and physicians
  • Compassionate, patient- and family-centered approach essential
  • Portuguese fluency a plus, but not required
