- Established physician-owned multispecialty group seeking a Nurse Practitioner to provide care in local skilled nursing facilities
- Competitive compensation — discussed directly with the client
- Full-time position serving SNF patients in the Fall River area
- Coordinated care model supporting transitions from hospital to SNF to home
- Focus on efficient, cost-effective care and reducing hospital readmissions
- Work alongside other NPs and facility medical directors
- Develop management plans and direct care for SNF patients
- Must hold active Massachusetts NP license, DEA, and MA controlled substances registration
- Strong organizational skills, independence, flexibility, and problem-solving abilities required
- Must collaborate effectively with nurses, therapists, pharmacists, administrators, and physicians
- Compassionate, patient- and family-centered approach essential
- Portuguese fluency a plus, but not required
