FHCSD Program for All-inclusive Care for the Elderly (PACE) is designed to help our patients maintain a high standard of independent living. Our program provides a wide range of supportive services including in-home care, meals, medical equipment, transportation, social services, and social activities. We are currently seeking a dedicated Nurse Practitioner or Physician Assistant to join our team at FHCSD PACE College Grove. In this position you will act as a crucial member of an integrated health team that is dedicated to providing well-coordinated and high-quality care to all of our participants.

Duties and Responsibilities
- Complete all required documentation in accordance with department standards
- Work effectively with patients from diverse social, cultural and economic groups
- Participate in PACE Interdisciplinary Team (IDT) meetings
- Maintain all necessary certifications and licensures required
- Attend all mandatory meetings; minimizing incident reports; using resources efficiently and practicing medicine in a cost-effective manner.
- Maintain FHCSD behavioral standards, including (but not limited to), effective listening and communication skills and respectful interactions with colleagues and patients.
- Maintain satisfactory peer reviews: able to assess medical information including pathophysiology, patient presentation, differential diagnosis, etc., in order to facilitate health promotion and disease prevention.
- Meet clinic site and organization needs in a reliable and flexible manner; prioritizes work and makes an effort to adhere to schedule.
- Participate in Clinical Outcomes Program and work towards meeting organizational targets for all metrics.
- Participate in administrative duties, including lab reports, charts, and medication refills reviewed and managed appropriately.
- Perform other duties as assigned.
Requirements
- Minimum of 2 years of experience working with underserved, low-income older adult persons in an outpatient clinic or similar setting required.
- Completion of an approved/accredited Physician Assistant or Nurse Practitioner program
- Current unrestricted license to practice in the State of California or ability to obtain
- DEA license or ability to obtain
- Current American Heart Association health care provider CPR (BLS), or Advanced Cardiac Life Support, Advanced Trauma Life Support, or Advance Resuscitation Training
- Must meet FHCSD Driving Requirements
- Second language fluency desired
