What You’ll Do

You’ll serve as a trusted clinical partner for patients managing chronic conditions — improving quality of life and reducing avoidable hospitalizations.

Clinical Responsibilities:

• Conduct comprehensive initial assessments and hospital transition visits

• Perform full vitals, head-to-toe exams, and hands-on evaluations

• Develop and manage individualized care plans

• Oversee medication management and adjustments

• Provide chronic disease education and lifestyle coaching

• Collaborate in team rounds for high-risk patient review

• Document in Athena with real-time visibility for care coordination

Visit Structure:

• 4–5 patients per day

• New visits: ~90 minutes

• Follow-ups: 30–60 minutes

Schedule & Flexibility

• Hybrid model: 3–4 in-home visit days + 1–2 remote/admin days

• AI-supported scheduling for optimized routing

• Monday–Friday schedule

• No weekends or on-call

Compensation & Benefits

• $120,000–$145,000 base salary (flexible based on experience)

What We’re Looking For

• Minimum 2 years of Advanced Practice Provider experience

• Experience managing chronic care plans preferred

• Passion for delivering compassionate, in-home patient care
