Job description

Nurse Practitioner – In-Home Care
Richmond, KY / Lexington KY

This Is Not a Clinic Job. And That’s the Point.

If you became an NP to practice autonomously, build real patient relationships, and actually improve outcomes—this role was designed for you.

We’re looking for a Nurse Practitioner who wants to step away from rushed visits, crowded schedules, and productivity pressure, and instead deliver thoughtful, evidence-based care to complex patients where it matters most: in their homes.

Why NPs Work Here?!

  • Practice at the Top of Your License – Independent, evidence-based decision-making with physician collaboration
  • Mission-Driven Care – Reduce hospitalizations, improve quality of life, and close real gaps in care
  • Time With Patients – Fewer visits, deeper conversations, meaningful impact
  • Schedule – 4 days in the field, 1 telehealth day from home
  • Support Without Burnout – You’re never alone, but you’re never overmanaged
What You’ll Do:
  • Deliver in-home and virtual care for patients with CKD through ESRD
  • Perform comprehensive assessments, wellness visits, and focused follow-ups
  • Coordinate care with PCPs, specialists, and an integrated RN + Social Work team
  • Educate patients and families, close gaps in care, and prevent avoidable hospitalizations
  • Prescribe, order diagnostics, document efficiently, and practice at the top of your license
  • Travel statewide during the workweek (up to ~75% travel)
Compensation & Benefits:
  • $120,000 – $140,000 base salary (depending on experience)
  • Comprehensive medical, dental, vision, life, and disability coverage
  • 401(k) with employer match
  • Paid holidays, flexible PTO, and paid parental leave
  • Mental health support, wellness programs, and growth resources
Qualifications
  • Active, unrestricted Nurse Practitioner license
  • Board certified (AANP or ANCC)
  • 2+ years of direct patient care experience
  • Comfortable working independently with patients in their home based environment
  • Strong clinical judgment, communication skills, and heart for complex care
The Difference You’ll Feel

This is a role for NPs who want their work to slow down, matter more, and align with why they entered healthcare in the first place. You’ll be trusted as a clinician, supported as a professional, and valued as a person, not just a provider!
