Travel Nurse Practitioner, Hybrid Schedule
Job at a glance
Job description
Nurse Practitioner – In-Home Care
Richmond, KY / Lexington KY
This Is Not a Clinic Job. And That’s the Point.
If you became an NP to practice autonomously, build real patient relationships, and actually improve outcomes—this role was designed for you.
We’re looking for a Nurse Practitioner who wants to step away from rushed visits, crowded schedules, and productivity pressure, and instead deliver thoughtful, evidence-based care to complex patients where it matters most: in their homes.
Why NPs Work Here?!
- Practice at the Top of Your License – Independent, evidence-based decision-making with physician collaboration
- Mission-Driven Care – Reduce hospitalizations, improve quality of life, and close real gaps in care
- Time With Patients – Fewer visits, deeper conversations, meaningful impact
- Schedule – 4 days in the field, 1 telehealth day from home
- Support Without Burnout – You’re never alone, but you’re never overmanaged
- Deliver in-home and virtual care for patients with CKD through ESRD
- Perform comprehensive assessments, wellness visits, and focused follow-ups
- Coordinate care with PCPs, specialists, and an integrated RN + Social Work team
- Educate patients and families, close gaps in care, and prevent avoidable hospitalizations
- Prescribe, order diagnostics, document efficiently, and practice at the top of your license
- Travel statewide during the workweek (up to ~75% travel)
- $120,000 – $140,000 base salary (depending on experience)
- Comprehensive medical, dental, vision, life, and disability coverage
- 401(k) with employer match
- Paid holidays, flexible PTO, and paid parental leave
- Mental health support, wellness programs, and growth resources
- Active, unrestricted Nurse Practitioner license
- Board certified (AANP or ANCC)
- 2+ years of direct patient care experience
- Comfortable working independently with patients in their home based environment
- Strong clinical judgment, communication skills, and heart for complex care
This is a role for NPs who want their work to slow down, matter more, and align with why they entered healthcare in the first place. You’ll be trusted as a clinician, supported as a professional, and valued as a person, not just a provider!
