πŸ’° $159,000 – $187,000 annually

About the Role:
We are seeking a skilled and compassionate Nurse Practitioner or Physician Assistant to join our integrated primary care team. This role focuses on delivering patient-centered, community-focused care, emphasizing preventive medicine, chronic disease management, and collaboration with behavioral health and wellness staff.

Key Responsibilities:

  • Conduct comprehensive assessments, diagnoses, and treatment planning

  • Prescribe and manage medications within scope of practice

  • Provide preventive care, primary care, and ongoing health maintenance

  • Perform physical exams, review vitals, and evaluate medical histories

  • Educate patients on disease processes, medications, prevention, and adherence

  • Collaborate with interdisciplinary care teams including behavioral health professionals

  • Refer patients to subspecialists as appropriate

  • Participate in after-hours on-call rotation

  • Maintain accurate, compliant clinical documentation

  • Contribute to team meetings, peer reviews, and quality improvement initiatives

  • Assist with coverage for other clinicians as needed

Qualifications:
Required:

  • Master’s degree or higher as a Nurse Practitioner or Physician Assistant

  • Active California NP or PA license

  • Current DEA registration

  • Minimum 2 years of clinical experience

  • Clinical background in Family Medicine, Internal Medicine, or Pediatrics

Preferred:

  • Experience in community health or integrated care settings

  • Knowledge of local, state, and federal healthcare regulations

  • Strong analytical, critical-thinking, and communication skills

  • Ability to work effectively in a multidisciplinary team

  • Bilingual skills required in one of the following: Mandarin, Cantonese, Spanish, Tagalog, Farsi, or Vietnamese
