About the Role:
We are seeking a skilled and compassionate Nurse Practitioner or Physician Assistant to join our integrated primary care team. This role focuses on delivering patient-centered, community-focused care, emphasizing preventive medicine, chronic disease management, and collaboration with behavioral health and wellness staff.
Key Responsibilities:

Conduct comprehensive assessments, diagnoses, and treatment planning

Prescribe and manage medications within scope of practice

Provide preventive care, primary care, and ongoing health maintenance

Perform physical exams, review vitals, and evaluate medical histories
-
Educate patients on disease processes, medications, prevention, and adherence

Collaborate with interdisciplinary care teams including behavioral health professionals
-
Refer patients to subspecialists as appropriate
-
Participate in after-hours on-call rotation

Maintain accurate, compliant clinical documentation
-
Contribute to team meetings, peer reviews, and quality improvement initiatives

Assist with coverage for other clinicians as needed
Qualifications:
Required:
-
Masterβs degree or higher as a Nurse Practitioner or Physician Assistant

Active California NP or PA license
-
Current DEA registration
-
Minimum 2 years of clinical experience

Clinical background in Family Medicine, Internal Medicine, or Pediatrics
Preferred:
-
Experience in community health or integrated care settings

Knowledge of local, state, and federal healthcare regulations

Strong analytical, critical-thinking, and communication skills

Ability to work effectively in a multidisciplinary team

Bilingual skills required in one of the following: Mandarin, Cantonese, Spanish, Tagalog, Farsi, or Vietnamese
