Our client, a reputable healthcare organization dedicated to comprehensive patient care, is hiring a Psychiatric Nurse Practitioner. This role offers an exciting opportunity to deliver ongoing, Follow-up care, including psycho-therapy and medication management, in a setting that emphasizes holistic, patient-centered treatment.

Responsibilities:

  • Provide follow-up mental health care for patients with psychiatric and behavioral health issues, including stabilization during early visits.
  • Conduct individual psychotherapy sessions targeting issues such as end-of-life anxiety, delirium, dementia-related behaviors, and dual diagnoses.
  • Collaborate closely with a multidisciplinary team comprising physicians, nurses, and a consulting psychiatrist to develop and implement individualized care plans.
  • Offer safety assessments, including close observation and intervention for at-risk or unstable patients.
  • Participate in on-call telephone consultations for weekend emergencies, with minimal on-site presence.
  • Maintain accurate documentation of patient interactions and treatment progress.
  • Support nursing staff and contribute to a coordinated, holistic approach to patient care.

Requirements:

  • Current valid certification as a Nurse Practitioner with a psychiatric specialization.
  • Minimum of 2 years of psychiatric nurse practitioner experience, preferably with inpatient or hospital settings and experience working with primary care teams.
  • Demonstrated proficiency in medication management and psychotherapy techniques.
  • Strong understanding of psychiatric conditions such as delirium, dementia-related behaviors, and dual diagnoses.
  • Excellent communication skills and the ability to work collaboratively within a multidisciplinary team.
