Were seeking a compassionate and skilled Nurse Practitioner (NP) to join a leading value-based care organization in Southern California. This role provides in-home patient visits allowing you to deliver truly comprehensive, patient-centered care to those that are homebound.

Base Salary: $135K$152K (commensurate with experience)
Bonus Eligible: Annual performance bonus program
Schedule: MondayFriday, no evenings, weekends

Your Role
Conduct health assessments and preventive care visits within established clinical guidelines
Manage acute and chronic conditions through evidence-based treatment plans
Collaborate closely with physicians and care teams to develop coordinated care plans
Order, interpret, and evaluate diagnostic tests to guide care decisions
Prescribe medications and therapies in accordance with scope and regulations
Provide education and empowerment to help patients proactively manage their health
Requirements
Masters degree in Nursing (MSN) required
Minimum of 1 year of experience as a Nurse Practitioner
Current and unrestricted RN and NP licenses in California
DEA license preferred
Passion for patient-centered, value-based healthcare
