Oklahoma City, Oklahoma – House Call NP/PA Job
Clinical Role:

Permanent position with a privately owned multispecialty group

Seeking a board certified or truly board eligible Nurse Practitioner (NP) or Physician Assistant (PA)
-
Home-based care: provide high-quality healthcare to patients in their own homes, focusing on chronic illnesses, post-hospitalization care, and patient-centered care
-
Multispecialty team collaboration
Key Responsibilities:
-
Patient Care & Clinical Assessment: Conduct comprehensive health assessments, diagnose and manage acute and chronic conditions, order and interpret diagnostic tests, prescribe treatments, and provide patient and family education

Chronic Disease Management: Manage conditions such as diabetes, hypertension, heart disease, COPD, and other long-term health issues

Care Coordination: Collaborate with physicians, case managers, and social workers to ensure coordinated care; refer to specialists as needed
-
Documentation & Compliance: Maintain accurate medical records in compliance with HIPAA, document patient care and follow-up, and ensure proper billing and coding

Acute Care & Emergency Management: Respond to acute health needs or emergencies during home visits and provide guidance on when to seek emergency care

Patient & Family Education: Educate patients and families on chronic disease management, medication adherence, preventive care, post-hospital care, and lifestyle modifications
Qualifications:
-
Master’s degree from an accredited NP or PA program
-
Current NP/PA license in Oklahoma
-
Board certified NP/PA
-
BLS required; ACLS preferred
-
Minimum 2 years of experience in primary care, urgent care, home health, or hospital setting preferred
-
Experience managing chronic diseases and home-based care preferred
-
Strong diagnostic, assessment, communication, and organizational skills
-
Proficiency with EHR (EPIC) and telemedicine tools
-
Ability to work independently while collaborating with interdisciplinary teams
-
Reliable transportation for travel to patient homes (mileage reimbursed)

Perform physical assessments and assist patients as needed

Occasional lifting or moving of patients or equipment
Compensation & Benefits:
-
Salary: $130,000 – $150,000 per year
