Join Our Team as a Nurse Practitioner - Palliative Care

Embark on a rewarding opportunity to deliver compassionate palliative care to patients in Medford, Oregon. As a vital member of our healthcare team, you will provide high-quality, onsite hospice and palliative services, ensuring comfort and dignity for patients during their most vulnerable times. Required Skills:
  • Valid active licenses in Washington (WA) and Oregon (OR), including DEA registration
  • Proven experience in hospice, palliative, or related clinical care
  • Strong clinical assessment and symptom management skills
  • Ability to collaborate effectively with RNs and interdisciplinary teams
  • Competency in medication prescribing and patient education
  • Excellent communication and interpersonal skills
  • Flexibility to participate in on-call and weekend coverage as scheduled

Nice to Have Skills:
  • Experience with telehealth delivery
  • Prior experience in home hospice or outpatient palliative care programs
  • Comfort managing a variable patient panel, approximately 15 patients monthly
  • Knowledge of electronic health record systems and telemedicine technology

Preferred Education and Experience:
  • Master’s degree in Nursing (MSN) or higher
  • A minimum of 2 years of clinical experience in hospice or palliative care settings
  • Current licensure and DEA registration for both Washington and Oregon

Other Requirements:
  • Ability to provide face-to-face care at the Providence Hospice location in Medford
  • Participation in symptom management calls from 5 PM to 8 AM on weekdays (must be reachable via beeper)
  • Potential weekend call coverage, averaging once per month
  • Willingness to assist with patient visits, telehealth consultations, and system management as needed
  • Guaranteed hours of 8 hours per day; no overtime expectation after 5 PM when on call
  • Travel or telehealth care for WA patients during call hours
