Family Nurse Practitioner (FNP) / Adult-Gerontology Nurse Practitioner (AGNP)

Location: Mesa, AZ
Job Type: Full-Time
Salary: $110,000 - $150,000 per year (DOE)

Job Description

We are seeking a Family Nurse Practitioner (FNP) or Adult-Gerontology Nurse Practitioner (AGNP) to provide comprehensive primary care services in an outpatient setting. This role involves delivering high-quality, patient-centered care through assessment, diagnosis, treatment, and ongoing management of acute and chronic conditions.

The ideal provider practices independently while collaborating with a multidisciplinary team and following established clinical standards and protocols.

Key Responsibilities

  • Provide primary care services to patients across the lifespan (FNP) or adult/geriatric populations (AGNP)

  • Assess, diagnose, and manage acute and chronic medical conditions

  • Deliver preventive care, health screenings, and patient education

  • Develop and implement evidence-based treatment plans

  • Prescribe medications and order diagnostic tests as appropriate

  • Maintain accurate and timely clinical documentation in the EHR

  • Coordinate referrals and collaborate with other healthcare professionals

  • Ensure compliance with clinical, regulatory, and safety standards

Qualifications

  • Graduate of an accredited Nurse Practitioner program

  • Certified as an FNP or AGNP

  • Active Arizona RN and APRN licenses

  • Current, unrestricted DEA registration

  • BLS / CPR & AED certification (AHA or Red Cross)

  • Ability to manage acute and chronic conditions independently

  • Proficiency with electronic health records

  • Strong communication and interpersonal skills

  • Cultural competence and commitment to serving diverse populations

  • Valid Arizona driver's license

  • Level One Fingerprint Clearance Card (required for certain locations)
