Position Overview
We are seeking an experienced Nurse Practitioner to provide comprehensive primary care to elderly patients in a PACE outpatient clinic setting with occasional local home/facility visits.
This role focuses on longitudinal care, chronic disease management, and quality-driven medicine — not volume-driven production.

(Occasional local travel within Monmouth County; mileage reimbursed)

Schedule
• Monday–Friday
• 8:00am–4:30pm
• 30-minute lunch
• Clinic closed on major holidays
• Light call only (triaged by RN first; NP rarely goes out)

• Start: ASAP (typically 2–3 weeks post-offer)
• Duration: 3 months, potential extension
• Type: Locums
• Setting: Outpatient clinic with occasional home/facility visits

• Geriatric patients (55+)
• PACE participants
• Mix of clinic visits, follow-ups, chronic care, and limited home visits
• Average 7–10 patients per day
• Panel-based care (not high-volume)

• Pay Rate: $60–75/hour (flexible based on experience)
• Mileage reimbursement: $0.725/mile
• No travel or lodging required (local candidates preferred)

Requirements
• Active New Jersey NP license
• Board-certified FNP, AGNP, or AGACNP
• Minimum 1–2 years NP experience in primary care (required)
• Geriatric experience strongly preferred
• DEA & BLS required
• Recent, hands-on primary care experience (not PRN-only)
