Job description
We are looking for friendly and skilled nurse practitioners to treat our geriatric patients with polychronic disease. Main focus is to improve the quality of life for our patients.
- Telehealth visits conducting wellness assessments and assisting patients with enrolling in benefits
- Ordering necessary tests and medications
- collaborating with patient's other providers
- Some in-home visits will be needed with some patients
Schedule: Monday–Friday, 8am–5pm
💼 Type: Perm
💰 Rate: Discuss by Phone + mileage reimbursement at $0.70/mile
👥 Patient Volume: 3–7 patients/day
🖥 EMR: Athena
- Active TX NP license, BC, BLS, and DEA
- 2+ years of direct patient care as an NP
- In-home care and geriatric experience required
- Nephrology experience is a big plus!
- 100% Malpractice Paid
