Provide Compassionate Primary Care to patients in Beaver Dam, WI with SSM Health

At SSM Health, we are committed to delivering exceptional healthcare services. Our outpatient clinic in Beaver Dam, Wisconsin, is conveniently located off Highway 151. We are seeking a compassionate Advanced Practitioner (FNP or PA) to join our Family Medicine team of 3 physicians and 5 Advanced Practitioners. New grads are welcome to apply.

Your Qualifications:

  • Collaborative, team player
  • Graduate of a nationally accredited Physician Assistant (PA) or Nurse Practitioner (FNP) Program
  • Ability to obtain PA/NP licensure in Wisconsin
