Nurse Practitioner – Primary Care

Victorville, California
Full-Time | Monday–Friday | 8:00 AM–5:00 PM PST

Position Overview

This is a permanent, full-time Nurse Practitioner opportunity created due to organizational growth. The role focuses on providing high-quality, patient-centered primary care to Medi-Cal and PAVE patients of all ages.

The position includes a mix of in-clinic, in-home (within a 30-mile service area), and telehealth visits. Flexibility across nearby regions may be required to support coverage needs.

  • Schedule: Monday–Friday, 8:00a–5:00p

  • Patient population: All ages

  • EMR: Athena

Primary Responsibilities

  • Manage patients with chronic conditions including (but not limited to) diabetes, hypertension, and CKD

  • Perform comprehensive health assessments and scheduled follow-up visits

  • Review labs, vitals, and symptoms; adjust treatment plans accordingly

  • Provide preventive care, lifestyle counseling, and disease management education

  • Support highly complex patients, helping them navigate the healthcare system

  • Deliver one-on-one, holistic, patient-centered care

  • Function both autonomously and collaboratively within a multidisciplinary team

  • Coordinate care between patients, families, physicians, and care team members

  • Administer appropriate tests and screenings

  • Ensure accurate documentation, HCC-focused coding, and compliance

  • Maintain strict HIPAA confidentiality standards

  • Perform other duties as assigned

Candidate Qualifications

  • Active California Nurse Practitioner license

  • Board certified NP (ANCC or AANP)

  • 2+ years of NP experience in Primary Care, Family Medicine, Internal Medicine, Geriatrics, or Palliative Care

  • Home care experience a plus

  • Strong knowledge of chronic care management and medication therapies

  • HCC experience required

  • Experience with Medi-Medi / dual-eligible or underserved populations preferred

  • Bilingual English/Spanish required

  • Active CPR/BLS certification

  • DEA license and prescriptive authority (post-hire acceptable)
