Specialty accepted: Home Health APP
Start date or shifts: As soon as credentialed – ongoing (6-month commitment required)
Daily schedule: Monday–Friday 8:00 AM–5:00 PM (8-hour guarantee)
Practice setting: Home Health
Types of procedures: Primary care, wellness exams, chronic disease management, population health, and kidney care
Number of patients: 3–6 per day depending on proximity
Ages: Adults
Additional information: Provider must be bilingual English/Spanish. Work involves home visits supported by at-home RNs and LCSWs. Credentialing typically 90–120 days (faster if enrolled with Medicare/Medicaid). Orientation includes a 3-day paid travel training at the national office. Client prefers full-time availability and candidates open to temp-to-perm opportunities. Mileage reimbursed at $0.44 per mile. Guaranteed 8-hour day.
