About the Role

Key Responsibilities
- Provide comprehensive primary care services to a patient panel of 15–18 patients per day
- Collaborate with interdisciplinary teams to develop and implement effective care plans
- Ensure smooth transitions of care with hospice, palliative care, and home care agencies
- Maintain accurate and timely documentation in the Gehrimed EMR system
- Graduate degree as a Nurse Practitioner or Physician Assistant
- Active and unrestricted NP or PA license in New York
- DEA registration
- Strong clinical assessment and diagnostic skills
- Excellent communication and interpersonal abilities
- Experience in post-acute care preferred, but not required (training available)
- Estimated compensation range: $130,760 – $154,911 annually
