Nurse Practitioner / Physician Assistant - Primary Care
Location: West Sacramento, CA
Job Type: Full-Time
Schedule: Monday-Friday | Day Shift
Setting: Community Health / Outpatient Clinic
Responsibilities :

Provide comprehensive primary care for acute and chronic conditions

Conduct patient evaluations, diagnoses, and treatment planning

Deliver preventive care, health education, and patient counseling
-
Collaborate closely with medical leadership and care teams
-
Manage a steady, structured patient volume with dedicated admin time
-
Utilize telehealth for a portion of daily visits
Important Note: Patient Volume & Workflow Expectations -
Average of 3 patients per hour (20-minute visits)

Includes two telehealth visits per day

Dedicated administrative time at the end of each day

Chart completion required within 72 hours

Ability to efficiently manage the in-basket, including patient messages, refills, and lab results

EPIC EMR experience highly preferred
Qualifications :
-
Active CA NP or PA license with board certification

DEA & BLS certification
-
1-2 years of outpatient, community health, or FQHC experience

Comfort working with multicultural and multilingual populations

Experience with EPIC EMR strongly preferred

Bilingual skills a plus (Spanish or other languages)
