We are a patient-focused primary care practice with 35+ locations across New York. Our collaborative, team-based model empowers Advanced Practice Providers (NPs and PAs) to deliver comprehensive, high-quality care in a supportive and collegial environment.

Position Overview:
As a Primary Care APP, you will provide independent, full-scope primary care services—including preventive care, chronic disease management, acute care, and wellness visits—for an established patient panel. You’ll work closely with physicians and care teams to ensure patients receive coordinated, high-quality care.

What You’ll Do:

  • Provide comprehensive primary care services to patients of all ages

  • Perform histories, physical exams, and evidence-based assessments

  • Diagnose and manage common primary care conditions; coordinate care for complex patients

  • Prescribe medications, therapies, and treatments within scope

  • Order, interpret, and follow up on labs, imaging, and diagnostic tests

  • Develop individualized care plans and promote preventive care and patient education

  • Collaborate with physicians, nurses, and care team members in a team-based environment

  • Document encounters accurately in the EMR and support quality metrics

Qualifications:

  • NP or PA degree from an accredited school

  • Board-certified in Family Medicine or Internal Medicine

  • Valid and unrestricted NY State license

  • DEA license (or ability to obtain)

  • Current BLS/ACLS certification

  • Clean malpractice history

  • Strong clinical, diagnostic, and communication skills
