Job description
Saint Mary of Nazareth Hospital in Chicago, IL, is seeking a dedicated and compassionate Nurse Practitioner to join our growing Primary Care team. This is a rewarding opportunity to provide comprehensive, patient-centered care in a well-established outpatient setting.
Position Details:
- Will be employed by Prime Healthcare Illinois Medical Group.
- Dayshift Schedule (Mon-Fri, 8 am to 5 pm).
- Full-time position in an outpatient primary care clinic.
- See patients for preventive care, chronic disease management, and acute visits.
- Collaborate closely with a team of physicians, APPs, and clinical support staff.
- Manage your own patient panel with opportunities for continuity of care.
- 40 hour/week schedule with minimal call responsibility.
Qualifications
- Spanish Proficiency Preferred
- Licensed and certified as a Nurse Practitioner (NP) in the state of Illinois
- Board-certified in Family Medicine, Adult-Gerontology, or related field Prior experience in primary care preferred but not required
- Strong clinical assessment, communication, and documentation skills

A reasonable compensation estimate for this role, which includes estimated wages, benefits, and other forms of compensation, is $110,000.00 to $125,000.00 on an annualized basis. The exact starting compensation to be offered will be determined at the time of selecting an applicant for hire, in which a wide range of factors will be considered, including but not limited to, skillset, years of applicable experience, education, credentials and licensure.
