We are seeking a Nurse Practitioner or Physician Assistant to join a collaborative, team-based outpatient primary care practice in New York. This role focuses on preventive care, wellness, acute visits, and chronic disease management for an established patient panel, offering an excellent balance of autonomy and physician support.
Job Responsibilities
✅ Provide comprehensive primary care services (preventive, acute, chronic care)
✅ Perform histories, physical exams, and routine wellness visits
✅ Diagnose and manage common primary care conditions
✅ Prescribe medications and treatments within scope of practice
✅ Order and interpret labs and diagnostic studies
✅ Develop and monitor individualized care plans
✅ Educate patients on preventive care and lifestyle modification
✅ Collaborate with physicians and interdisciplinary care teams
✅ Document encounters accurately in EMR
Qualifications
✔️ NP or PA degree from an accredited program
✔️ Board Certified (Family Medicine or Internal Medicine)
✔️ Active and unrestricted New York State license
✔️ Valid DEA license or ability to obtain
✔️ Current BLS/ACLS certification
✔️ Clean malpractice history
Working Hours
🗓️ 4 or 5-day workweek (regular clinic hours)
